I just added the Water Rescue Extension for my fire department, but after some time, I am not getting any water rescue calls. I checked the missions tab to see if I wasn't doing anything wrong, and everything seems to be in order—stations, requirements, etc. I also noticed that POIs were needed, so I placed some around the station where the Water Rescue unit is. Having completed some 150 calls, I have yet to receive even one water rescue call. If anyone can help me figure out what I might be doing wrong or enlighten me on the spawn rate of these calls, I would be much very greatful!

I am sure one of the moderators will chime in here, but I believe that the number of water rescue missions directly relates to the number of extensions. I believe that missions are generated randomly by some formula. In other words having only will not generate a lot of missions. This is how it was explained to me when I questioned the lack of "airport" related missions despite having 5 or 6 airports.

Do you think making it a Specialization would help it to generate? I have gotten a Wildfire Extension, then a Specialization and I have gotten a lot of Wildfire calls.

Calls are generated at random, realistically unless you have an abundance of water rescue stations, they are not common calls to spawn.
Let's say for example, you have 300 stations, 100 fire, 100 police and 100 EMS. So right there, we have a 1 in 3 chance of spawning a fire mission. Now, if you only have 2 water rescue extensions, that means that out of the 33% chance of a fire call, that is subdivided into a 1% chance of the 33%.
If you further subdivide it into the fact that the station is NOT specialised, then that drops even further (as stations with specialisation will also spawn regular missions).
That is a very broad look at it, just to bring it into perspective.
I have been receiving water rescue calls recently, so I believe it is still working.
