If we can put some of the PT information from the call in there for more authenticity and maybe more information from which call this patient is from? When transporting other patients from other calls it usually happens in rapid succession where you don't really know who's who and who's going where.
Seeing as you provide pt details in the original call details, why not forward them onto the transport team. Maybe the ems call got worse or like in real life sometimes you get dispatched to a difficulty breathing and then they are a full arrest or completely fine and are a lesser acuity patient.
