The items highlighted in yellow are obviously very clear. What does the blue highligh mean? Also, can someone clarify if building fire stations and adding the ambulance extension counts toward being able to purchase MCI?

Well it's telling you that per every 20 Ambulance stations you can buy 1 mass cas unit. Then the text you have in blue is telling you that if you build 4 more ambulance stations then you will be able to purchase 1 mass cas unit.

Bottom one is:
"You are at 16 / 20 stations built, build 4 more to unlock another mass cass" -
Also yes, ambulance extensions count towards the mcu purchase.
