I've had this issue in the form of vehicles showing up without personnel. I don't know if that's what happened with you, though.
No mine had 2 staffed but just didn't assist the patient not showing as on scene until I cancelled it and dispatched it again.
Like Faolan said, this bug happens when the ambulance arrives at the scene without any personnel on it, are you sure your ambulance had staff? Because that would be a really interesting bug

Positive. All my ambulances are assigned staff. Maybe it was a one time thing.
I've had trucks with personnel assigned to them still show up empty on-scene. It's not a very frequent thing for me, so it shouldn't happen too often for you either. Still a bit frustrating, though.

All my units have the full crew assigned, as a matter of fact I wait until i have the exact crew members until I buy a vehicle for that station, and this has happened to me many times, mainly on ambulances because it's a lot easier to

Only about 25% of my calls are EMS, 25% Police, And the rest are Fire Dept calls.
If I have any specialized units I either assign them or make certain half or nearly 3/4 are trained in that specialty.

I understand, I only noticed this issue when I began expanding at Portugal, since I only had EMS stations I began to see this frequently
At the current time since I have a big part of my calls coming from NY and at Portugal I also have Police and Fire (in much smaller propotions compared to EMS)
I have 52 EMS stations just at Lisbon with a total of 151 ambulances 
I have had this EXACT, I suppose BUG happen like others, never with the "Staff not showing up"
But ALWAYS with the Ambulance being Dispatched
I watch it travel and arrive at destination and clearly says Ambulance WITH trained staff is there
I'd say 95% of the time the Ambulance does not register so can not collect ANY patients.
I DO have the relevant Skills trained at Hospital, Traumatology, General Internal and General Surgeon and 2 DEDICATED staff for EACH ambulance, so this isn't a Training and/or Staff bug.
This means I either have to ( as others also mentioned ), cancel ONLY the Ambulance and then re-send that unit OR
try Using the " RESET AAO Button " which also never seems to clear the problem either, as one player said could work, at least it did for them.
This ONLY happens with the Ambulances, no problems with Fire / Police / Sheriffs and/or Specialist Units
Even my logs show 1 ( ONE ) mission in 65 sent out during that entire days session was able to collect their patient and transport them to Hospital.
That kind of BUG needs fixing.
