So I built a Clinic, maxed out expansion, and added 2 ALS Fire Engines. Of course, these trucks require special training. I also have all academies. The problem is the clinics do not show up in the academies to training the personnel.

You built Clinic today or before? i mean do tou have any workers in there, because clinic comes with 0 at beginning.
But in general for me it would be strange thing to add Fire Engine/Ambulance to clinic. It is (Fly car + Fire engine) and NOT (ALS + Fire Engine) how you stated.

As far as I was aware, the ALS Engine can transport patients and thus is in fact an ALS Ambulance & Fire Engine, not a Fly Car & Engine

EMS Fire Engine/Ambulance it is called and in Missionchief Wikia page it states it is Fly-car/Fire engine.
You are saying it is ALS ambulance/ Fire engine instead? And that page has wrong information?
I cant imagine which fire engine with ambulance equipment vehicle with ambulance personnel can transport patients to hospitals in real life.

2. Not selectable for "Patient Transfer missions" does not mean that the vehicle is unable to transport from an incident, it just means it is not selectable to transfer patients from one facility to another.

Sorry, just clarify I bought 2 EMS Fire Engine/Ambulance and I have 12 staff. There is no place to train the clinics in the Academies for the ALS Medical Training for Fire Apparatus.

I’ve no doubt you’re aware of what it does - my additions to the thread were to help REALAIS understand their use. I agree that it’s a bug or an inconvenient omission by the devs and needs to be fixed.

No, I don't believe the devs monitor this regularly if at all. But the Admin and Content Creation Team usually mention major bugs to the devs. Best way to report bugs though is through the contact us section of the App Support.
On a side note - forum rules are not to double-post, so might be worth keeping that in mind otherwise the admin team usually step in
