More calls like (CPR, Unknown Medical, etc.)
Maybe even fire alarm activations, Lift Assist, and the like. It's really hard when several major missions stack up, consuming every last resource.

I don't really have much to say about your other stuff quite frankly I can't be bothered to argue, but for the record there are places where the ALS fire engine is a thing... So that thing "everyone knows" isn't exactly true xD
Wasn't arguing, just know that when a person new to the forums (perhaps not the game) comes on and talk about how things should be and "not liking certain features", it doesn't sit very well with the developers, administrators, and advisors who put a lot of free time into what the game is and does..

If you take a second to google it instead of assuming you know all, you'll see it is realistic. I love that there are so many people on here who voice their interests and preferences for the game, as long as they don't disrespect the substantial effort put into the game by volunteer content advisors and the other quite reasonable and non-presumptuous suggestions of respectful members of the community.
As for your actual suggestions, they add new missions every Monday, and there's a great thread on the US Forum for suggesting missions, which will then usually be picked up by the content advisors and passed to the devs. It's worth remembering though that the USA is a very very large country, and there are bound to be huge differences at local, regional and state levels for how often types of calls take place, what the response procedures are, the apparatus used etc and the game simply cannot replicate those differences per player geographically, so there will always be certain calls, units and mechanics which aren't going to fit your area.
My opinion is that it's better to add more generalised and broad-reaching content first, and more specific content later down the line when most of the general stuff is covered, as this benefits the majority of players. I also think with units it's better to add new ones than change or remove existing ones, as this gives players more opportunity to create a fleet suited to them. Personally, I see no use for the ALS Engine in my setup as it currently exists, however I would definitely like to see an engine being able to act as a fly-car , perhaps with ALS trained staff as a requirement for it to do so?

here are some examples of combination Ambulance fire engines, they are rare, but they exist.

friend of mine in Orange City FL. works for Volusia County, told me they have 5 of those engines and have more on order
As others have stated already, transporting fire engines exist (i personally think theyre stupid but my opinion on the matter doesnt change reality) hence why they're in the game. Now from a gameplay standpoint, theres only about 5 missions total that I ever send the 4 ALS Fire engines that I have to which makes them pretty useless to me. I find that if I send them to calls with a lot of patients, they want to transport before the mission is done and I end up needing to send an engine in its place while the mission is finished up which is very slow and gets very annoying. However, they have been useful in the past.
Engines and trucks could double as fly cars but then why not just get an ALS ambulance, you make more money by transporting than you would if you had a BLS fly car engine. Yes it is realistic that all firetrucks in the US (per NFPA I believe) have to have some form of an AED and some very basic life saving equipment, but its a video game and I really dont think we need that many multipurpose units.
Also why fewer fires? Is it all that realistic? FDNY got around 40,000 fires (27,000 structural fires and 13,000 non structural fires) in 2018, they are going to 109 fires a day... So is it really all that unrealistic to have multiple fire calls in one game "day"?

A reminder that Fly-Cars act as ALS upgrades to BLS units, meaning that if the vehicle acts like a fly-car, it would have to transport to hospital with the BLS ambulance, when acting as an upgrade vehicle.
At present those vehicles DO act as "First Responders", in the sense that they will start patient care on patients until an ambulance arrives (treating approximately 10%).
As another poster mentioned, if the vehicle acting as a fly-car leaves the scene for patient transport, it will cause the progress bar of the mission to stop and will require that the vehicle requirement be met again.
So is there any point to getting BLS units other than RP? I can see the reason for a fly-car, but I still don't know why I would get BLS.
There are lot of "A" and "B" patients that show up, especially in the "patient transfer" missions. I use a BLS unit in the Clinics to transport those facility type patients to keep from using and ALS unit.. Otherwise, I just put all my fire based ems units as ALS. If I build and EMS station somewhere, I usually just put one BLS unit it it.

that is the only reason I went with the BLS Ambulances was for RP, my rescue squads are the EMS Rescue units since our FF/Paramedics do perform technical rescues. It was pretty expensive to make my account as RP 
..
