With the recent hems update shouldn't this not require the base to spawn especially since the chopper being needed has been removed

I kind of want to see HEMS being required at more of the major incidents. In most systems, HEMS attends major incidents to fill the Medical Advisor or CCS Medical Lead roles rather than to treat individual patients.
