I have had the same 5 patients in my hospital since midnight last night it is currently 530 in the evening. I have a hospital with 17 beds and I have not been able to use 5 of them since midnight. This bug is happening with all my hospitals but the one I noticed it first on and timed it with is my largest

Was the hospital full at one point. Hospitals will only release one patient per hour, regardless of when the arrived. So if you send 17 patients to your hospital at the same time, it will take 17 hours for the hospital to fully clear (without anyone else going to the hospital).
If the hospital is shared with an alliance, that includes any patients sent to the hospital from the alliance as well. So if one patient is sent to the hospital (6 patients), the next hour only one patient will clear (back to 5 patients).

I can manually release but I don't like the fact I will have to spend coins to open up my hospital instead of the timer

i've had this with prisoners. You have to contact support to get them to do anything. They just manually release them for you.
