What about alliance hospitals? Alliances cannot own vehicles, only buildings.

Alliance Hospitals could allow players to send Ambulances to that hospital until they decide to transfer it somewhere else. The Alliance Admin would be the one to use alliance funds to expand ambulance bays for the members to send them there. The only problem would be alliance members fighting over space unless they contributed for more space.

Alliance Hospitals simply won't have access to the expansion, only to hold patients, problem solved.

Let's not? That's being quite selfish to all the other users. Not EVERYONE is based in the US, we could make some kind of adjustments to where it fits both US and other parts of the world? Some US Hospitals have ambulances, or if you don't want them, simply do NOT buy the expansion, simple.

I personally think it shouldn't be for alliance. it should be for people who want the extension and they already have a hospital

Not sure if I get you correctly. The entire alliance would run the ambulances then it would be a good idea.
I was simply saying let multiple players post ambulances at an alliance hospital.
But your idea sounds good too.
