I've got a HEMS with 254,094 miles on it right now as #1, and 2 ALS ambulances right behind it with 227,572 and 214,935 miles.

September of 2019, and usually between the 30 second mission spawn or 20 seconds.
I have been playing almost a year and play normally at the "normal" speed, but on the weekends when I am bored and more points are available for missions, Turbo.

All depends - Nomrally i play on 30 second spawns, but when the game decides to constantly throw majors at me, i have to stop and clear it all otherwise all my vehicles end up the wrong end of the country
I have 561 vehicles (in the middle of a fleet redo) and my most used vehicle is an SRU at 39.6k miles, which is quite funny
