Als update for fire trucks

  • I would love to see a update to the fire engine ambulance where there’s a a option to allow it to not transport just begin healing and require an ambulance to transport or a separate class to allow fire engines to have true first responder capability also to allow ems chiefs to begin patient care before the ambulance arrives. I play based on Philadelphia Pa where all engines are bls providers in some cases Als and it would be great to replicate that in the game.

  • He is thinking about an option to allow the EMS Engine/Ambulance to transport or just act like a fly car (I am guessing they mean like a BLS Upgradable (IE requires a fly car (or other equivalent unit) or ALS Ambulance to fully treat a patient))

yes I know, I wish they are able to act as ALS engines as a lot of departments are going that route, and a few departments in SoCal do Engine follow ups with the ambulances

Exactly what I am thinking the engine operates as a BLS engine and begins initially treating the patient to a BLS or ALS level however it still needs an ambulance to transport. A BLS level would be more realistic.

  • So the ask is to change the EMS Engine/Ambulance to NOT transport?

I think he is talking about having BLS and ALS patient types for calls, all fire engines would be able to treat BLS but not transport (so assume its like a lift assist, a BLS run but likely will end up with a refusal and thus doesnt always need an ambulance). ALS patients would need an ALS ambulance or some sort of engine with ALS providers.

Honestly, and this is a personal thing, I really dont like the inclusion of EMS transporting fire engines, in the game acting as both a firetruck and a transporting ALS unit, it gets in the way with a ton of shit, especially when there are patients at a call that requires lots of enignes, it ends up getting in the way when i go through my transport requests and accidentally send that engine to the hospital and thus require another engine from somewhere else, or I would need to consciously choose not to send it, and then just get another ALS unit to the scene which by that point I could have just sent 1 ALS unit and 1 engine rather than the ALS transporting engine. It eats up assets instead of freeing them like squads and quints (attempt) to do.

If it were up to me, and the way I read this chain, all engines should be able to treat BLS without an ambulance, and ALS transporting engines should be changed to just ALS treatment but no transport (in essence, treat the patient fully and then have it come in as a transport request, even better, have it tell you beforehand that transport will be needed. Example is cardiac arrest, an ALS engine could start working on them, get them prepped for the ambulance, and when the ambulance gets there they just stick the guy in the back and cook it all the way to the hospital).

I kinda went off on another tangent of a topic but let me know if I got that wrong, and what people think about that.

  • See an example on this thread:

ALS Medical Training for Fire Apparatus

This vehicle is based on the real life apparatus that is a transporting ALS fire engine.

  • So the ask is to change the EMS Engine/Ambulance to NOT transport?

I think the most appropriate implementation of this right now would be to add an "ALS Equipment" to the equipment market that can be added to Engines, Trucks, Quints, etc. that requires the ALS Training (min 2 personnel) to function and in turn would add fly car capability to that equipped vehicle.

Changes to the EMS Engine/Ambulance seems like it is more targeted as a workaround than as an actual request.

Yes, South Ogden Utah has one, it definitely serves a purpose in real life but I do not think that it is necessary to have in the game. Actually i think it is a hindrance to have one and I probably should have just bought regular transporting ALS ambulances rather than 4 ALS transporting engines, I never use them because they are super impractical and cause issues with patient transport and mission requirements.

  • That’s exactly what I mean except i would make all engines basic life support and still require an ambulance to transport patients. And yes their are transport capable fire trucks but at least in the USA they are extremely rare I have only ever seen maybe 2 dozen online and none that I know of in my area of Pennsylvania EMS-Chief

Absolutely that would work add it to the available equipment that actually is a lot more simple. But another suggestion would be to increase storage capabilities of engines and ladders to 20 so they can still carry say rescue equipment. EMS-Chief

  • I have a friend with the Greenville City FD in NC that I talk to often he's at station 7 where their engine/ambulance is based out of. He tells me they rarely transport the transport portion is available when there are no other transport available but they are trying to phase out their normal T engines and bring in more engine/ambulance combos. I have seen a mini pumper/ambulance back east that looks like an extended ambulance T1 with the pump panel on the specialist side of the rig. Personally, I don't use the combo in game as I try to play realistically on both my accounts in the So Cal area.
