is it best to have everything in 1 dispatch or multiple dispatches? do larger size runs create if all the stations are running off the same dispatch? or is there a cap on limits of high end runs are off a hand full of stations?

Depends on how you want to play. I use irl dispatches for counties and place all the stations in that county under that dispatch.

All depends on how you wish to play, but bare in mind there is a limit to a dispatch center for every 25 buildings. I personally just use 1 LA County (for LACoFD & McCormick) 1 for LA Fire, 1 LA Police etc but it's your game

I just wasn't sure if there are more major runs that generated after having all the combined stations in 1 dispatch or it doesn't matter?

Some of the "Major" missions (i.e. Passenger Train Derailment (Major)) require 40 ambulance stations. If you have the create your own coverage area option turned on, those missions wouldn't spawn in areas covered by dispatch centers with less than 40 ambulance stations/expansions.

the only dispatch center I have that option turned on is the Catalina Island area , 2-FDs, 1PD, 1 Clinic and 1 hospital on the same dispatch

This has so many ambulance stations in game as a need as it has a max of 60 patients on on the uk game a max of 150
